Rewards Programs
- Every $1 token purchase earns you 1 point towards our loyalty rewards program.
- There is no time limit, you can earn a reward everyday or you can let your points build up to select a larger prize and/or multiple.
- You should wait AT LEAST 48 hours before reaching out for an update.
- Forms may be filled out once every 24 hours
- Disagreements with the rules or causing drama over the program may result in being banned from the Loyalty Rewards Program
- This rewards program is based off of how much you load
- We have three levels you can redeem at:
Reward Level

1. Every 500 points = $50 Amazon Gift card or Applied to 1 Platform of your Choice
2. Every 1000 points = $100 Walmart, Amazon, Target Gift card or Applied to 1 Platform of your Choice
3. Every 1500 points = $150 Gift card of your choice! (e-gift card), or Applied to 1 Platform of your Choice

- You MUST meet the minimum point level to submit this form
- You MUST include your Facebook name AND a method of delivery for the gift card on the form
- Rewards will not be delivered instantly, it can take up to 7 business days to receive it.
- You MUST play through your loyalty rewards if you select a load to a platform - a buyback cannot be immediately requested

To qualify you must have loaded at least one time during the year BEFORE your birthday
This form must be filled out AT LEAST ONE WEEK PRIOR TO YOUR BIRTHDAY.
Each person is allowed ONE per year on the day they submitted the form for -- PLEASE NOTE this is a free gift, we do not have to do it and we can take it away if it causes drama
You may be asked to provide a copy of your ID with your name and birthday if any questions or issues arise.
Attempting to use fake accounts to gain extra birthday bonuses will get you permanently banned from Slot Luck
Amount of the birthday load is at Slot Lucks discretion and is subject to change.
